Your First Visit

When you arrive, you’ll be welcomed by our team and asked to complete:
- A Health History Form
- General contact and insurance information
- Review and acknowledgment of our Privacy Policy
To save time, ask us how to complete forms in advance.

Payment Information
For Uninsured Patients:
- Cleanings, exams, and minor treatments: Full payment is due at the time of service
- Major treatments (e.g., crowns, restorations):
- 50% due on the first day of treatment
- Remaining balance due on the final visit

We also offer flexible payment options:
- Monthly payment plans for qualifying patients
- Financing available through CareCredit and Sunbit

Appointment & Cancellation Policy
We’ve proudly served the Oak Lawn community for over 60 years and continue to value every patient’s time. To ensure availability for all patients, we ask that you follow our appointment policy outlined below.
Arrival Policy
- Please arrive on time for your scheduled appointment.
- If you arrive more than 20 minutes late, we may need to reschedule to stay on track for other patients.
Cancellations & Missed Appointments
- Appointments require at least 24 hours’ notice to cancel or reschedule.
- Saturday appointments require 48 hours’ notice due to high demand.
Missed Appointment Fees
- Weekday Appointments:
$50 per 30-minute block (Dr. Smith or hygienist) if canceled or missed with less than 24 hours’ notice - Saturday Appointments:
$60 per 30-minute block with less than 48 hours’ notice
These charges apply to each half hour of reserved appointment time.
If you need to reschedule, just call us in advance — we’re happy to help you find a new time that works.
